Monroe Capital LLC announced it acted as sole lead arranger and administrative agent on the funding of a senior credit facility and equity co-investment to support the strategic growth of Vytalize Health (“Vytalize”), alongside private equity sponsor, Enhanced Healthcare Partners.

Based in Hoboken, NJ, Vytalize facilitates and manages risk-based contracts for primary care providers serving more than 250,000 patients. Vytalize acts as an intermediary between CMS and the provider practices, offering evidence-based recommendations at the point of care and assisting in the generation of savings through a data-driven approach which includes monitoring patient health, providing wraparound services such as in-home care and in-patient monitoring, and engaging with providers to develop plan of action and preventative care for its patient population.
